BEFORE PARTICIPATING IN OUR ATHLETIC AND DANCE PROGAMS AND UTILIZING THE VARIOUS FACILITIES THAT WILL BE USED AS PART OF OUR PROGRAM, you must first acknowledge and agree to our policies as stated below. Disclaimer
You should always consult your physician before starting any exercise program or changing their diet. By enrolling and participating in any classes offered by the California Athletic Club, you have acknowledged that you understand the risks of injury that may incur by participating in any fitness program or class such as those offered by the California Athletic Club. You also acknowledge that using materials and/or equipment by provided by the California Athletic Club in any other manner than it is intended for absolves the California Athletic Club of liability for injuries should the materials and/or equipment malfunction or break.
You should always consult your physician or other health care professional before starting this or any other fitness program to determine if it is right for your needs. This is particularly true if you (or your family) have a history of high blood pressure or heart disease, or if you have ever experienced chest pain when exercising or have experienced chest pain in the past month when not engaged in physical activity, smoke, have high cholesterol, are obese, or have a bone or joint problem that could be made worse by a change in physical activity. Do not start this fitness program if your physician or health care provider advises against it. If you experience faintness, dizziness, pain or shortness of breath at any time while exercising you should stop immediately.

Billing
NOTE: This policy is required. If you do not accept it, you will not be able to enroll this student into any classes or camps.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.
Rules, Terms, and Conditions
NOTE: This policy is required. If you do not accept it, you will not be able to enroll this student into any classes or camps.
Enrollment: Priority enrollment for the following session is offered to our enrolled families during the 7th week of the session. Taking advantage of priority enrollment will help ensure your spot in the class. Open enrollment (open to non-enrolled students) will be available during the 8th week of sessions 1-5 (call office for open enrollment dates for Summer Sessions). Your child’s spot is not guaranteed until a payment is received.
Tuition: A yearly $40.00 registration fee will be due upon registering to cover the cost of secondary insurance for you. We will charge this fee annually. This fee is non-refundable.
▪ If starting mid-session, you will be pro-rated.
▪ During our 8 week session, a payment plan is available. You may make a half-payment at the beginning of the session, and second half is due at the 4th week of the session. You are still obligated to the full 8 week session. All ½ payment plans must include a credit card on file. You may pay the ½ payment via cash or check before the first day of the 5th week. If you do not make your half payment during the 4th week, we will charge your credit card on file for the second half payment.

There are no refunds once a student starts their first class: When you pay, we are reserving your spot in class, therefore we cannot give refunds. If personal situations/injuries keep the student from attending more than 50% of the session, please notify our office immediately. Failure to do so will result in you being responsible for tuition until we are notified that your child is temporarily or permanently dropping class. Keeping up to date records is difficult without knowing what your intentions are. Help us help you by maintaining class size integrity.
Returned Checks: There is a $25.00 fee for returned check.
Make Up Policy: Make-ups are offered by appointment only on Fridays. 2 make-ups per 8 week session and 1 make-up per 5 week session. Due to class ratios, we may not always be able to offer a make-up class. You must schedule these make-ups with the front desk. Make-ups can only be scheduled after the class has been missed and must be made-up in the same session. No make-ups for a missed make-up class. There are no credits or refunds for missed classes. You must check in with the front desk when you arrive to pick up your make-up slip. We work very hard to promote consistency for our students and coaches. This is very important to the learning process.
DROP PROCEDURE:
ALL MEMBERS MUST NOTIFY THE SCHOOL TO DROP A STUDENT FROM CLASS. Only a written notice via email, regular postal mail or hand delivered to our front desk will be acceptable.
Please note: You are responsible for payment for your classes WHETHER OR NOT YOU ATTEND CLASS until the time you notify the staff VIA WRITTEN NOTICE. Please do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list.
What to Wear: Athletes must abide by The California Athletic Club dress code at all times. The California Athletic Club reserves the right to refuse service if dress code is violated. Women in the soccer program are required to wear gym shorts, sweat pants, T-shirts, shin guards, socks, and soccer cleats. Yoga pants. Boys may wear comfortable shorts or sweat pants with a T-shirt. Hair must be pulled back off shoulders and away from the face. For safety precautions, NO JEWELRY IS TO BE WORN DURING CLASS. All students will be barefoot during class.
Arrival: Be sure you arrive 20 minutes before your scheduled class time. During peak times the parking lot is crowded. Please take into consideration the time you may need to find parking, receive treatment, and prepare for the start of class. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.
Late to Class Policy: If you are late to class by 10 minutes or more, please check in with the the lead trainers to receive a late slip. This is to ensure that you will be sent to the proper place in the warm-up, plus help us keep attendance records correct.
Extended Absence: If you are unable to attend 2 weeks or more due to a medical problem, we will pro-rate that session’s tuition for you. Please notify us immediately. (DOCTOR’S note is required) regarding your situation. This is for your safety and well being! Please let us know prior to returning to class so we can regulate class sizes and to help keep up to date records.
Testing: All enrolled students will be tested in week 1 and 8 during regular class time in Sessions 1-5. During Sessions 1-5, each student will be automatically evaluated on skills found on their skill chart. Remember that Men's Football. Women's Soccer, and Woman's Dance are difficult sport that requires skill, strength and flexibility: all of which can take many months and years to develop. Additional academy testing will be by appointment only at a charge of $10 (semi private) or $20 (private). Testing is scheduled on a first come first serve bases either during open gym on Saturday (semi private) or throughout the week (private). See the California Athletic Club Website for testing times.
No regular class testing during the Summer Sessions: A & B. Summer time testing will only be done via private testing.
Holidays: We are closed on some holidays; please check the California Athletic Club Website or call the front officee to confirm (323) 722-1133 those dates. Reminders will be handed out to students.
Food: No food or drink of any kind (except water) is allowed out in the training area.
Lost and Found: Jewelry, any valuable items and money will be kept at the front desk. Clothing/shoes/bags/water bottles/etc. are placed in the blue lost and found container in the gym.

DISCLAIMER: THE CALIFORNIA ATHLETIC CLUB IS NOT RESPONSIBLE FOR ANY INJURY (OR LOSS OF PROPERTY) TO ANY PERSON WHILE PRACTICING, TAKING CLASS, COMPETING, PARTICIPATING IN OPEN ATHLETIC TRAINING AND CONDITIONING CLASSES, OR IN ANY OTHER WAY INVOLVED IN ATHELTIC TRAINING AND CONDITIONING, CHEERLEADING AND DANCE CLASS, TEAMS AT THE CALIFORNIA ATHLETIC CLUB FOR ANY REASON WHATSOEVER, INCLUDING ORDINARY NEGLIGENCE ON THE PART OF THE CALIFORNIA ATHLETIC CLUB., ITS OWNERS, OFFICERS, AGENTS, OR EMPLOYEES.
Under my own legal authorty, I hereby consent to participating in the facility's that will be utilized for this program(s)., I hereby release and covenant not-to-sue THE CALIFORNIA ATHLETIC CLUB., THE CALIFORNIA ATHLETIC CLUB'S Board of Directors and officers, THE CALIFORNIA ATHLETIC CLUB, and any of their employees, teachers, coaches, or agents, from any and all present and future claims resulting from ordinary negligence on the part of THE CALIFORNIA ATHLETIC CLUB., or others listed for property damage, personal injury, or wrongful death, arising as a result of my engaging in or receiving instruction in Men's Football, Women's Soccer, gymnastics, cheerleading, Women's Dance, or any other activities or any activities incidental thereto, wherever, whenever, or however the same may occur. I hereby voluntarily waive any and all claims resulting from ordinary negligence, both present and future that may be made by family estate heirs, assigns, or me.
Further, I am aware that Men's Football, Women's Soccer, Women's Dance, and cheerleading, are vigorous sporting activities involving height and rotation in a unique environment and as such they pose a risk of injury. I understand that Men's Football, Women's Soccer, Gymnastics, Women's Dance, Cheerleading, and related activities always involve certain risks, including but not limited to, death, serious neck and spinal injuries resulting in complete or partial paralysis, brain damage, and serious injury to virtually all bones, joints, muscles, and internal organs, and the mats, pits, and other safety equipment and apparatus provided for my protection including the active participation of a coach or teacher who will spot or assist in the performance of certain skills, may be inadequate to prevent serious injury. The risk of harm may be limited by all of the safety equipment and trained coaches, but never eliminated. I understand that participation in Men's Football, Women's Soccer, gymnastics, Women's Dance, Cheerleading including moving from event to event, conditioning, stretching and other activities which may leave me vulnerable to the reckless actions of their participants who may not have complete control over their actions or who may not see other students in the gym. I am voluntarily participating in this activity with knowledge of the risks involved and hereby agree to accept any and all inherent risks of property damage, personal injury, or death.
I give my permission and consent for a licensed doctor or physician to administer the necessary aid to me should I become injured or sick while in attendance at or while participating in any activity associated with THE CALIFORNIA ATHLETIC CLUB and do so without having to wait until.
I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation and exposure.
I hereby release, discharge, and covenant not to sue your business (THE CALIFORNIA ATHLETIC CLUB ), it's administrators, directors, agents, officers, volunteers, employees, contractors, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of the premises on which the activity takes place, (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, damages, on my account caused or alleged to be caused in whole or in party by the negligence of the RELEASEES or otherwise, including negligent rescue operations and further agree that if, despite this release, waiver of liability, and assumption of risk, I or anyone on my behalf, makes a claim against any of the RELEASEES, I will indemnify, defend, and hold harmless each of the RELEASEES from any loss, liability, damage, or cost, which any may incur as the result of such a claim. I understand that this waiver in intended to be as broad and as inclusive as permitted by the laws of the state of California and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect.
I further agree that the venue for any legal proceedings shall be within the state of California.
I affirm that I am of legal age and am freely agreeing to this agreement. I have read this form and fully understand that by agreeing to the statements on this form, I am giving up legal rights and or remedies which may be available to me for the ordinary negligence of THE CALIFORNIA ATHLETIC CLUB, or any person listed above. if, despite this release, I, or anyone on my behalf makes a claim against any of the above RELEASEES, I WILL DEFEND, INDEMNIFY, AND HOLD HARMLESS each of the RELEASEES from any litigation expenses, attorney fees, loss liability, damage, or cost which any RELEASEE may incur as the result of any such claim.
Photo Waiver

Security cameras recording all events are in place throughout THE CALIFORNIA ATHLETIC CLUB practice facilities . Participant further permits THE CALIFORNIA ATHLETIC CLUB to photograph and/or video tape for promotional use.
